Primary Health Network Commitment to Community Results in Target: BP Gold+ Achievement Award

Sharon, PA – To improve community health outcomes regarding high blood pressure, also known as hypertension, Primary Health Network (PHN) committed itself to meeting the strategic standards of the American Heart Association (AHA) and the American Medical Association (AMA). PHN is proud to announce that its commitment to the community to impact blood pressure and its potential negative effects has yielded positive results: Primary Health Network has received the Target: BP Gold+ achievement level award.

Awardees of the Target: BP Gold+ achievement presented by the AHA and AMA accomplished an unprecedented amount of 70% or greater blood pressure (BP) control rates with the adult patient population served and attest to at least four of six evidence-based blood pressure measurement criteria.
