In addition to this prestigious recognition, PHN has also received its Patient-Centered Medical Home (PCMH) certification, further underscoring its unwavering commitment to providing high-quality, patient-centered care to the communities it serves.
The Joint Commission, an independent, nonprofit organization, sets rigorous standards for healthcare organizations to meet in order to achieve accreditation. Since its initial accreditation in 1997, PHN has consistently demonstrated its dedication to upholding these national standards. Each time a team of Joint Commission surveyors conducts a comprehensive onsite evaluation, PHN has proven its ability to meet and exceed expectations, ensuring the highest level of healthcare delivery.
