Primary Health Network Achieves Continued Accreditation in Behavioral Health and Ambulatory Care, Receives PCMH Certification

Sharon, PA – Primary Health Network (PHN) is proud to announce that it has once again achieved full accreditation in both Behavioral Health and Ambulatory Care from The Joint Commission on the Accreditation of Healthcare Organizations (The Joint Commission). In addition to this prestigious recognition, PHN has also received its Patient-Centered Medical Home (PCMH) certification, further underscoring its unwavering commitment to providing high-quality, patient-centered care to the communities it serves.

The Joint Commission, an independent, nonprofit organization, sets rigorous standards for healthcare organizations to meet in order to achieve accreditation. Since its initial accreditation in 1997, PHN has consistently demonstrated its dedication to upholding these national standards. Each time a team of Joint Commission surveyors conducts a comprehensive onsite evaluation, PHN has proven its ability to meet and exceed expectations, ensuring the highest level of healthcare delivery.

A Leading Provider Across 17 Counties

Primary Health Network is proud to be the largest Federally Qualified Health Center (FQHC) in the region, providing care to over 75,000 patients across 16 counties in Pennsylvania and 1 county in Ohio. Each evaluation has validated PHN’s continued commitment to excellence and its ongoing efforts to improve the quality of care it delivers.
